This webinar examines women’s sexual function in the context of gynecologic brachytherapy through anatomical, dosimetric, and survivorship perspectives. Dr. Deborah Marshall will discuss the clinical significance of accurately contouring bulboclitoris and its role in female sexual function. Dr. Stella Lymberis, MD, will review bulboclitoris dose considerations in brachytherapy for low-lying vaginal cancers, and Jennifer Bires, LCSW, OSWC,CST will address sexual health recovery following radiation for pelvic malignancies, highlighting communication strategies and evidence-based support. Join us to learn the key elements of an MR-only guided workflow for gynecological brachytherapy. Priscilla Dreyer, Medical Physicist at the Juravinski Cancer Centre, will walk through the complete process—including recommended MR sequences, techniques to improve applicator visibility in MR imaging, and strategies for identifying applicators while accounting for distortion in MR images.
Because applicators can appear distorted on MR, many clinics still rely on both CT and MR for planning. CT introduces far less distortion and provides clearer visualization of applicators. However, results from the EMBRACE II trial underscored the importance of MR-guided brachytherapy for cervical cancer, prompting more radiation oncology centers to consider MR-only planning for HDR cases.

This Varian-sponsored luncheon at the October 2025 ABS workshops in Palm Desert addressed a 2025 white paper on “Selection of Hybrid Applicators for Adaptive Cervical Cancer: A Practical Guide.” Four prominent radiation oncologists, Cody R. Kilar, Harjot Kaur Bajwa, Phillip M. Pifer, and Sushil Beriwal authored the publication. The EMBRACE II trials demonstrated that image-guided brachytherapy and particularly MR imaging are vital in the treatment of cervical cancer because of optimal dosimetry and critical organ sparing. Hybrid techniques use needles to supplement traditional cervical applicators allowing the dose to be adapted to the clinical case and sparing critical structures. The talk addresses which applicators may be used in different clinical scenarios as well as needle selection and how many, with considerations for anatomy and tumor size.
